Membership
Below is the information needed in completing the Chapter Membership process. Please follow the steps below to secure your membership.
Note: You must have Microsoft Excel installed on your computer to complete the forms. No mailed copies will be accepted, please email forms only.

Steps to Submit Chapter Membership List
STEP 1
Download the Chapter Membership Excel Document.
STEP 2
Save and name the form “Chapter Membership List” on your computer.
STEP 3
Fill out all portions of the form.
As you work on the form, hit “Save” frequently
so as not to lose your work.
Associate Members, including men, should be listed at the end of the form.
STEP 4
Attach the completed form to an email and send it to:
Membership@Philoptochos.org.
Please note your Chapter Number on the Subject (Re:) Line of the email.
STEP 5
Please use a coupon to send in your membership per capita.
STEP 6
After you have completed the membership steps for National Philoptochos, please use the form below to mail in your Metropolis of Chicago per capita check along with your check for the Bishop’s Welfare Fund.
